We Get To Know You
During your first visit, we will discuss your dental history, take images of your teeth and mouth, and perform a comprehensive exam. Once we an understanding of your current dental situation, our team will create a personalized treatment plan with you and answer your questions..
We accept all major credit cards as well as Care Credit and HSA. At this time we are not in-network for any insurance carriers. We will help you fill out the paperwork to submit to your insurance company so that they can reimburse you the appropraite amount.

What forms of payment are accepted?
All forms of credit and debit cards including Visa, Mastercard, Discover, and American Express are accepted along with cash or check.
Do we accept third party payers?
Yes we accept FSA and Care Credit.
How do I schedule my preventive care?
Simply call our practice and reserve your appointment time for yourself or your family members.
What happens if I miss an appointment?
It will be your responsibility to reschedule. If you should need to cancel your appointment, we ask that you give 24 hours notice. If 24 hours notice is not given and/or you fail to show for your appointment, a fee may be applied to your account.
What information is required for me to sign up for my Concierge Plan Membership?
Full name, address, phone, date of birth, and an e-mail address.
When can I start using my Concierge Plan?
You can begin using your concierge plan effective immediately.
How can I use my Concierge Plan for other treatment?Simply reserve your appointment time with our practice and be sure to tell us you are a member of our plan. When you arrive, we will check your membership status you will be eligible for instant savings off of our regular fees.
Can I add members to my plan?
Absolutely! Your entire family can join.
Can I transfer my Concierge Plan membership to another dentist?
Your membership is solely with our practice and as such is non-transferrable.
